Take the application form, and with a magic marker write in large letters, &quot;I Have Opted OUt.  Do Not Send Anymore Mailings.&quot;  Leave your address visible so they know who you are.  Then put it into the postage paid envelope and add as much junk paper as you can fit along with something rigid is good, too, and seal it into the envelope and drop it in the mail.  The company will be charged by the weight of the envelope, plus it will incur an extra non-machineable fee.  Good luck getting rid of the buggers!</description>
