Filing Lawsuit Relief?

In the Sample Basis for Lawsuit in book Good Credit is Sexy on page 83. At the bottom it says:

Wherefore PLAINTIFF prays for the following relief:

State the relief you are seeking be sure to include actual, statutory, and punitive damages.

Would someone give me an example of what to put here...

This will depend on the facts of your case, however in many states you can be general:

1. Actual damages in an amount to be proven at trial (or not less than $__);

2. Statutory damages pursuant to (list statute);

3. Punitive damages;

4. Such other relief as the court deems proper.
