dispute/Affidavit of Fact? HELP!!

I disputed an account (first card/first usa bank) with the credit bureaus. Apparelntly they forwarded the information to the OC. Now I received a letter from them that states:

We received your dsipute from the credit reporting agencies, reagarding the infor reported for the above referened account. After researching your dispute, including a review of our records related to the account, we were unable to clearly determine your responsibility for the account. Therefore, we infromed the credit reporting agencies that the account should be reported as disputed on your file pending a further investigation. O.K> now the rest..If you wish to continue to dispute the reporting of thie account, we will be aksthat you send a signed copy of the enclosed affidavit to the following addres...blah..blah..Once the info has been received someone from our Fraud Dept will contact you. Then they enclosces a sheet titled :affidavit of fact fradulent credit application..they want us to sing that we never applied for a card, show our signatures, provide ssn #'s, telephone number addresses and fill out a part stating who we believe is usitilize the account.. Sorry this is so long, but I've never heard of this and don't think it sounds like something I want to fill out sign and send back. Anyone else have this happen? How should I proceed? They already said they can't verify it in their letter. Why would I want to send more info?

I disputed with them as well, and they sent me several copies of that affidavit...like every three or four days for a week. I did not fill it out and am still disputing with them. I have not recieved any calls from them. Dispute it with the CRAs. I sent them copies of my letters to the OC and will keep disputing with them until it is removed.

Unless the OC sends this affidavit certified mail,they have no way to determine that you received it.
