For CRA purposes, It starts at the first due date. If hospital policy is pay upon service, it's the date of service. If policy is 30 days, it's 30 days after service. Find out what the hospital policy is regarding when payment is due or use the first bill you ever got, or a copy of the contract.

For SOL purposes, I think it's the same, but could be the date of service regardless of when it became due, since there was never a payment made, the last transfer was them providing you service.
