When the CA sends incorrect validation.......

I got a letter from a CA yesterday. Included with the letter was a computer printout of transactions. The CA did not supply me with ANY PROOF that I asked for in the 1st DV letter I sent 20 days ago. Is there a sample letter to send to them for not providing me with proper proof? If not, what is my next step?
