Name difference cause for inaccurate information/removal

I have a old negative charge off from 1999. However, the name on the account is not my actual name that the social security office has on file. (this happened while I lived at a guardian's [not to be confused with adoption] residence and was under the assumption from them that my name had been changed to match their last name). Should I dispute with the CA's that the information they have concerning the last name is incorrect? How should I go about this?
