FNBM sent me a letter of affidavit after disputing

I received a letter of affidavit from FNBM yesterday after disputing with Experian. Should I fill it out. Thing is, I know that the account is under a differant last name than that of my current legal name and the old name does not appear on my credit report. Plus when they sent me the "standard response" letter it was address to my current name with a bad middle innitial. So i know they are just passing the buck on this one. Should I fill out the affidavit or call the company and play dumb to get more info. Or is this not even a correct form of Validation? How do i proceed?
