Addresses for disputing with CRA's

I am hoping this letter is sufficient to sent to the CRA's and that the addresses i have listed for them below are correct.

To Whom It May Concern:

I am contacting you in regards to an item listed on my TransUnion Credit Report:

Account # Amount

Midland Credit Management XXXXXXXXX $135

I have no recollection of this account and do not believe it to be mine. Please validate. Or remove it from my Credit Report.

*****Also, if TransUnion is showing this Midland Credit Account in the amount of $135 owed and the other two CRA's are showing an amount owed of $134. Should i state that in my letters to the CRA's??
