Long story short she lost her purse with all the goodies in '03. Someone then applied for 2 credit cards in her name, and maxed em out. You know the story from here, charged off and into collections. Many consumers use the FTC affidavit because of the convenience of the form, but the law only calls for a 'statement'.The list of required docs is found in 1681c-2, subsection 6605B.The definition of, and reason for, a proper ID Theft report is found at 1681a, subsection 603(q).
