This letter is being sent to you in response to reviewing my Credit Report. Be advised that this is not a refusal to pay, but a notice sent pursuant to the Fair Debt Collection Practices Act, 15 USC 1692g Sec. 809 (B) that your claim is disputed and validation is requested.................

Meh, it works.

There are a few "Upon reviewing my credit report, it has come to my attention that you have placed a derogatory listing on EQ, EX and TU, indicating that I do not pay my bills." letters floating around here.

Wording it like that is basically calling them out and has a more serious tone that one politely asking why they would do such a thing.

I believe in being more direct with a nod to the FCRA:

This letter DISPUTES, per the state and federal FCRA, inaccurate, incomplete or false information you are reporting, to wit:

a.XXXXXXXX- not my account

b.YYYYYYYY - not late 8/06

c.ZZZZZZZ - closed at my request, please so note

Please remove them and send me a letter telling me you have done so. This invokes my rights under the FCRA.

Don't be shy about giving them the last 4 digits of your SS or your address. They do need it to match you up with their records.
