Interesting Dunning Letter

I received an initial dunning letter from Wells Fargo today that contained the usual demands. However, it had a form attached to it with instructions to fill out and return:

1. Name, Address

2. Cell Phone number

3. SS#

4. Employer

5. Position

6. Work Number

7. Bank Account Number

I’ve never seen this before. It makes me wonder how often people actually return something like this?
