Help understanding CMR?

I just received the Case management forms. Equable Ascent sent me bare bones documents in response to the request of BOP. I sent in 2 more requests without knowledge except for a form stating the document was already sent. Do I file the motion to compel with the court house and the Case Management statement at the same time?

BUMP.... update:

I received a summary/request "Declaration of plaintiff in lieu of personal testimony at trial" from a name I never heard. Included was a summary of the case and copies of the the cardholder's agreement with a circled clause, bill of sale from Chase, and 1 statement.

Trial is set for about a month away, what can I do?
