Got a mediation hearing after filing for a more definite statement! Help!

Hi,

I posted before that i was sued for a cc debt of approx $3000 in Ohio to which I filed a motion for a more definite statement by reading these forums. I finally received a response which included a few copies of account statements (not many, maybe 10-12), and the Plaintiff's memorandum in opposition to my request for a more definite statement along with an amended complaint including said account statement copies.

It looks like now I have a mediation hearing set in a couple of weeks. I am not sure what to do. I would like to take the arbitration route but I cannot find a Chase Cardholder agreement for the Disney Rewards card. Also, I am unsure of when this card was opened and they did not give me anything that shows when it was opened either. My guess would be around 2006-07.

Can someone help me find a cardholder agreement for this? I would like to request arbitration but I am not sure how. I also would like to do it before my mediation hearing because I do not know if in my State going to mediation waves my rights to arbitration.

Please help. Any help would be greatly appreciated.
