Motion to Vacate in CA

Need some help here but hopefuly not much. I had my accounts levied by Razor Capital II LLC via Nelson & Kennard by way of default judgment. Since I never received notice to appear in court this came as a surprise. I have learned that I should file Motion to Vacate but got confused. Am I now the plaintiff on the form or the defendant? Its asking for dollar amounts on the plaintiffs end and if that is me do I fill in the amount the judgment was set for or the amount they have levied thus far? This is for form SC-100 in CALIFORNIA http://www.courts.ca.gov/documents/sc100.pdf
