Is this a "proper" Bill of Sale?

That identical format has been used to secure judgments against consumers for ages.  You can hope you get a judge that will entertain the "anyone could have created that exhibit" argument, but as of late, that argument has been falling flat.

Your best shot is to get the case into arbitration if this option is available to you (read your card agreement to find out).

Notice the contract and document Id's listed at the top of bill-of-sale. They reference the rest of the documentation. Some folks will tell you to try to get them to provided the entire sales contract, etc., but that argument falls on deaf ears, because you are basically wasting the court's time in an effort to uncover even MORE evidence against you.

@Harry Seaward @Goody_Ouchless @calawyerisn't this the very 'evidence' what you would call heresay? Don't you request live witness testimony against this sort of 'evidence'?

This is exactly what a jdb law firm sent me, along with a couple statements and an out of state affidavit from an OC employee. 

Is this all a jdb needs then to win in court?

CA has friendlier rules in this regard.   It's your CCP 96 or CCP 98.   The CA posters can give you that information.
