Is there something I need to do to get some feedback? Am I missing any details?

I just want some guidance from the experts if I just respond to the claim (have never been served) and start the fight or if I just ignore it until I'm served?

Is this for an old bank account or a credit card?

Someone from FL will have to chime in. I've never seen hearings for a case when no one has been served.

Does a substitution of counsel requires the presence of both parties?

The hearing on 8/29 I panicked and send a Motion for Continuance and Notice of Unavailability to then find out this was a hearing for substitution of counsel.

Until today, I have yet to be served. Can I file a motion to dismiss for failure to serve? (I believe its been over a year already) and also for Failure to Attach since no agreement was attached on the original claim?

Read your rules of civil procedure.  They will tell you the amount of time a plaintiff has to serve a complaint after its been filed.
